Transitional Care Services is recognized by insurance companies in terms of excellence, proven results and high satisfaction levels. 

We accept Midicare and Medicare Replacement plans. These include but are not limited to traditional Medicare, United Healthcare Secure Horizons and Evercare, Anthem Senior Advantage, Humana Medicare and Aetna Medicare. If you are enrolled in a Medicare Replacement plan not listed please contact our Director of Admissions to inquire if Transitional Care services at Garden Manor is a provider under your plan. We accept most Medicare Replacement plans.

Transitional Care Services is also a Medicaid provider and is contracted with a multitude of Commercial Insurance carriers. Garden Manor is approved by the following Commercial Insurance carriers:
