Discharge Planning

The goal of Transitional Care Services at Garden Manor is to get you home quickly.  Our team of clinical experts works in collaboration with the team of medical professionals involved in your care to start the discharge planning process immediately upon your admission.  We recognize that there is no place like your home, but premature discharges may result in safety issues, re-hospitalizations and other medical complications.  Therefore, continuous communication regarding the discharge planning process occurs throughout your stay.  This process also involves key elements critical to your success at home such as family/caregiver education and training, home evaluations to promote safety and adaptations, community-based programs and arrangements for home based services.

Discharges are arranged to promote smooth transfers from Transitional Care Services to home.  The discharge planning process also allows you the opportunity to stay in close contact with your medical specialists, and physicians and community based service providers.  Count on our team to be available for you following your discharge to answer questions, provide assistance and obtain valuable feedback regarding your stay.
