Our rehabilitation services focus on helping you reach your highest level of independence as quickly and safely as possible!

Individualized post-acute support and personalized physical, occupational and speech therapies are provided within a unique environment to enhance recovery and healing.

At Transitional Care Services, our therapy team is comprised of licensed therapists and licensed therapy assistants with advanced training and expertise in the programs we offer.  Our therapists work directly with your surgeon, physician and medical staff to create a personalized program that is designed around you!

A 1,600 square foot rehabilitation gym was remodeled by our therapists and opened once again in January of 2011 to smooth your transition from the hospital – to Garden Manor – and back to home….where you want to be!

Our programs consist of the following:

• Initial evaluation and establishment of rehabilitation goals
• Ongoing communication with physicians, specialists and insurance providers
• Therapeutic exercises for range of motion and strengthening
• Ambulation and mobility training for safe navigation in the home and community
• Pain Management
• Individual and family education
• Instructions in self-care, household management and community based activities
• Adaptive and supportive equipment training
• Comprehensive discharge planning and coordination of care with social worker
• Evaluation of individual’s home prior to discharge can be conducted
• Recommendations for follow up therapy, equipment and services as necessary
• Personalized and detailed home exercise programs
